Go to Quality Process Designer > Designer.
- Select Forms from the dropdown.

Click on the New + icon.
- Complete the Basic Details of your Form.
- Click on Next to go to Form Configuration.
- Reorder your Fields, if necessary and configure your form as required. (Please note it is not possible to re order Groups or Field Types.)
- Add Triggers, if required.
- Save.
