Go to Quality Process Designer > Designer.
- Select Fields from the dropdown.

Click on the New + icon.
- Configure mandatory fields as described in How to create a Field in QPD, then Select Radio buttons from the Field Type dropdown.
- Add a Label.
- Add a Value.
- Add/ Remove Labels & Values.
- Select Numeric or Text.
- Toggle on/ off Mandatory for this Field Type.
- Save.
